Reward Card Term Conditions

1. The Kofsky Optometry Reward Card and Family Reward Card are issued by and remains the property of Kofsky Optometry Pty Ltd. Kofsky Optometry Pty Ltd reserves the right to withdraw the card at any time. 2. Kofsky Optometry reserves the right to alter or amend the conditions of the operation of the Reward Card and Family Reward Card, or terminate the schemes at any time. No alteration, amendment or termination of the scheme will affect the reward point entitlements issued at the time of any alteration, amendment or termination of the schemes. 3. All participants in the Reward Card must be resident in Australia. 4. The Reward Cards cannot be used as a credit or guarantee card. 5. Each new member joining the scheme will be rewarded with $25, this value is only redeemable once a basic eye examination has been attended at Kofsky Optometry by each member or a minimum spend of $250 by each member at Kofsky Optometry, 789 New South Head Road, Rose Bay. 6. To qualify for Kofsky Optometry Reward Points, one point is earned for every one dollar spent. 7. All reward points are only redeemable on optical frames and sunglasses. 8. The redemption value of points may be changed at the discretion of Kofsky Optometry. Any changes shall not affect points issued prior to the date of change. 9. Bonus points will be offered from time to time during special promotions on selected merchandise and can be earned in addition to normal Reward Card points. 10. Reward points are not transferable to another non-family member. 11. Kofsky Optometry ensures the integrity and privacy of Reward Card members’ details are maintained and upheld at all times and not disclosed or sold to any other organisation for any reason.
