Joining is easy!

1. Fill in your / family details on the membership form.
2. We will provide you with your Reward Card or Family Reward Card(s) by post in about 2 weeks.
3. To activate your card please book your appointment(s) for a basic eye examination, which is fully or partially claimable from Medicare. A Kofsky Optometry staff member can advise you accordingly of your entitlements for each family member where applicable. Alternatively, your Reward Card or Family Reward Card will be activated with a minimum spend in-store.
